LivingWell@Home (Home & community)


LivingWell@Home is a remote patient monitoring service designed to help you take proactive steps to maintain and enhance your health – without leaving home.


The service is aided by technology and includes telehealth equipment that helps identify changes in your vital signs.

A team of nurses and data specialists confidentially review your information gathered from the LivingWell@Home technology, and will alert your home care agency with any concerns about unusual changes in your vital signs.


You can stay in control of your health by identifying potential health risks, have peace of mind through early detection and intervention, and live as well and as independently as possible—with the assistance of LivingWell@Home and caring staff members.
